Lucy did a great job of working her way through her monthly assignment list. However, she was also asked to help with a few other residents.
Reporting
Lucy ensures that she has recorded the outcomes for each ACP attempt and discussion in the electronic medical record.
- She also follows-up to ensure all ACP documents are in place and that the staff who take care of each resident are aware of any new information
- She reviews her outcomes with her supervisor at the end of the month. They discuss her progress and identify some processes that need to be fixed. A plan is created to educate incoming nursing staff
- A new structured assignment list is generated. Lucy gets to work!

At the next Quarterly Assurance Meeting, Lucy is asked to report outcomes for the prior month
